I have a form on my site for requesting a trail package, but when a user wants to fill out this form, they get an error.
And my form styling gives an error: (I translated it): “EXAMPLE: There is a problem with sending. The errors are highlighted.”
I have attached what I see.
Maybe is my site hacked or even maybe is Formidable hacked?
Can you help me, please?
My site is http://gezondechocola.nl

Formidable hasn't been hacked. What you are seeing on your screenshot is perfectly normal. The screen you are displaying in the screenshot is for styling purposes only. That error message is an example error message that a user might see if they fail to enter information into a "required" field. You can customize the appearance of this error message from this page.

When I fill out something in that field, it won't be remembered / updated?
For example, I filled in (translated in English) "This is required field, please fill out"

On the settings page (not the styling page) there is a section for default messages, you can edit the error message there (see screenshot). once saving your those settings, the change should be reflected on your forms, but the "styling" page from your screenshot above won't be updated text.
