The easiest way to do this would be to add a custom field on the page you are including the form on. This custom field would contain the email address to send to. Then, you can add a hidden field to your form with the dynamic default value [post_meta key=whatever]. Change "whatever" to the name of your custom field.
Then, in your email settings tab, check the box for your hidden field.
