Is it possible to get the same form with Formidable pro as the excel form in attachement ? and the store in database the same data structure

in the historic field.

Can you try uploading that file as an xls file extension instead of xlsm? I tried opening it up in google docs but it couldn't open the file.

When I preview the file on my mac it appears to be a basic information form, so I think I am missing something. I don't have excel installed on my computer, so I need something that is viewable in google docs or in preview.

Or if you would like to explain what you are trying to accomplish, we can try to point you in the right direction.

These calculations look a bit beyond the capabilities of our plugin. But it is difficult to tell from that screenshot. The best thing I can recommend is to try this out in and let us know if/where you run into problems. That way you can provide a link to your form and we can take a closer look at what you have and are trying to do.
