Custom template does not import

I had two separate forms that I am now trying to combine.  Rather than rebuild, I exported the templates and combined them into one PHP file.

I have checked for contractions and unclosed ‘… but all seem to be preceded by .  Template loaded once, but in the incorrect order.  I changed field_order and updated via the settings page.  Still not showing on my Custom Templates display.  I have attached the template,  any idea what’s causing this?

Path is: /html/wp-content/form_templates

Can you please zip the template and attach it here? And also, what is the example path under the template path setting?

Example path is:

/home/content/50/8238150/html/wp-content/plugins/formidable/classes/templates

I have tried the example path and it doesn't work either.

First, use the path /home/content/50/8238150/html/wp-content/form_templates

Let me know if that still doesn't work and I'll take a look at the template when I'm at my computer.

That worked!  However, had to deactivate and re-activate the plugin after updating the templates.  Thanks for your help.
