creating a single checkbox line

How do I create a single checkbox item in a form (such as ‘tick this box if you wish us to contact you’)?
When I drag the checkboxes field into my form it automatically populates the form with two checkboxes (option1 and option2). I can’t seem to find anyway to make the second unneeded option disappear. Any tips?

When you hover over the second check box a small trash can appears beside it. Click that and it will disappear. Alternately click bulk edit options under the check boxes and delete the second box title.

Thanks Bob, but unfortunately neither of those solutions seem to be available to me. I have tried both your suggestions in Chrome, IE9 and Firefox and in each case there is no trash can appearing next to the second field and when clicking on the bulk edit options in the case of Chrome and Firefox a blank shadowbox window box opens and in IE the shadowbox opens with a message saying that the website cannot display the page. Any other thoughts?

Sounds like you have a theme or plugin causing conflicts. Please go through and deactivate these, starting with your theme.

Also, what version of Formidable are you running?

Stephanie, I am running 1.06.00 which I know is not quite the latest (1.06.02). Do you think updating the version might possibly solve this problem? The site I am working on has quite an old base version of Wordpress (2.8.4). We are intending to upgrade the base version in about a week's time to bring it right up to date. Do you know if 1.06.02 will still work with 2.8.4 in the meantime if I just upgrade the version of Formidable?

Upgrading might fix the blank bulk edit popup since we removed some hooks other plugins were using to cause trouble. It won't resolve any javascript errors on the page. Have you gone through deactivating your theme and plugins to find the conflict?
