Most everything is running nicely now with my instrument rental forms. There’s just one issue. The confirmation email meant for the customer isn’t always sent.
We tested 6 forms about 12 times each, so out of 72 times, there were two instances where the confirmation email was not sent.
Also, I have 3 email addresses entered in for the confirmation email to us admin. I seem to always get the confirmation email when a customer submits a form, but on the trumpet form, one admin is not receiving the confirmation email. I can’t see any differences between the setup of other forms and the trumpet form, and also, I don’t know why I’m receiving the confirmation emails when one other admin is not.

Formidable Pro uses the same Mechanism to send mail that WordPress uses, so we don't have a whole lot of control over this, but here are a few things to try. http://formidablepro.com/knowledgebase/why-arent-i-getting-any-emails/
I highly recommend using an SMTP plugin and setting it up to send mail via SMTP rather than via PHP mail. (there is a link to a recommended plugin on the page linked to above).
