This drives me nuts. I have a form that are supposed to send the form entry to one mail adress, and also to to send a copy of the entry to the mailadress entered in the form, the “and/or” field. The entry mail isn’t sent to the recepient but the customer that entetered his mail adress in the form gets the copy.
I have WordPress 3.3.2. and Formidable Pro 1.06.03.

Is the reply to/from email the same as the recipient email? Some email hosts have issues if this is the case. Try changing the reply to/from email address, does that change anything? You may need to install an SMTP plugin. We recommend: WP Mail SMPT
There are a few other suggestions to try here, but if those submitting the form are getting the emails, I am guessing the SMTP plugin is your best bet: http://formidablepro.com/knowledgebase/why-arent-i-getting-any-emails/

Hmm, If nothing comes through from the check email plugin, that means WordPress isn't sending mail, which is strange because the emails sent to the user filling out the form are. I would definitely install the wp smtp plugin and make sure it is sending mail as SMTP in the settings.
