Formidable is hands down the best, most innovative and easy to use form builder for WordPress I've ever used. Calling this plugin a simple "form builder" is a bit of an understatement - people with no knowledge of HTML, CSS or programming can create extremely complex forms with ease.

You could just make a checkbox that is required, right?

Yeah, most people add a paragraph field, and add their terms as default text into this field. Then they add a required checkbox field to allow them to "agree" with the terms you have added to your form.

Oh, and you would also want to set that paragraph field as read-only (field settings) so users can't modify the text.
