Add form in Modal and pass variable

Trying to load a form into a modal by clicking a link on this page http://datamyne.ifwstaging.com/free-report-library/

1. How would u load the modal using pretty photo?

2. How would I add the proper URL to pass the report value?

note I added the link <a target=”_blank” href=”http://datamyne.ifwstaging.com/index.php?plugin=formidable&controller=forms&frm_action=preview&form=8wgxlt&report=22“>TEST FORM/a> at the bottom of that page. The form loads but doesn’t pass the parameter. I’m also using the default value for the report field [get param=report].
